Youth in Transition Grant

Our Mission:
Young adults, families/adult allies and community partners collaborate to develop a system of care to support transition to adulthood. The system builds upon the strengths of young adults and creates an array of specialized mental health and related services to meet their unique and changing needs. The system also fosters young adult leadership.

Opportunities:

The E-Team:
The E-Team is an evaluation advisory group made up of young adults and parents or guardians. E-Team members can come from anywhere in Vermont. The E-Team meets 4 to 6 times per year and helps to inform the evaluation of the Young Adult in Transition system of care initiative. Ways the E-Team shapes the evaluation ranges from deciding what outcomes to measure and how to measure them, to how do we share what we are learning with Vermont communities.

CLC Committee
The CLC Coordinator has created a CLC Workgroup to assist child, youth, and family serving agencies within the System of Care to implement culturally and linguistically competent practices. So far, YIT staff and young adults from four regions are participating in this group: Chittenden (Burlington), Franklin-Grand Isle (Saint Albans), South Windham (Brattleboro), and Orange North Windsor (Hartford). The CLC Workgroup will meet monthly at different locations throughout Vermont to stimulate discussions about CLC among YIT staff and key stakeholders (including young adult and family leaders) in all regions.

Social Marketing Committee

The Social Marketing Committee is dedicated to making the system of care for this population be more young adult friendly. This group helps make decisions on marketing campaigns used around the state. Together this group has the power to change the state with their creative ideas.
